Researchers at Harvard Medical School have identified a potential new connection between immune activity in skull bone marrow and chronic pain conditions. The discovery builds on earlier findings that revealed tiny channels linking the skull’s bone marrow directly to the brain’s protective lining, suggesting the skull functions as an independent immune response hub.
In a study examining brain scans from 125 adults experiencing chronic pain—including back pain and knee osteoarthritis—scientists found elevated levels of a protein called translocator protein (TSPO) in skull regions compared to healthy control subjects. The protein, which is abundant in immune cells, showed the strongest signals in participants with knee pain and correlated with reported pain intensity and daily life disruption.
Tissue analysis from deceased donors provided additional evidence. Skull bone marrow samples from an individual with chronic pain contained twice as many cells as samples from a person without pain history, with a higher proportion of TSPO-positive immune cells. While researchers acknowledge the mechanism remains unclear, they propose that immune signals may trigger a cascade of neuroinflammation stemming from skull bone marrow activation.
The findings suggest that targeting skull bone marrow through therapeutic approaches could potentially offer new treatment avenues for chronic pain sufferers. However, scientists emphasize that additional research is needed to determine whether this immune activity causes chronic pain or represents a consequence of it.
